How Often Should Skin Peels Be Undertaken?

Skin Peels may be undertaken 2 to 4 weekly, depending on the peel type. Lactobotanical peels can be done fortnightly, while more active peels (advanced treatments) should be done monthly.

Are Peels Safe During Pregnancy?

The great news is that radio frequency devices like the Scarlet S are suitable for all skin types, even darker skin types, for skin tightening. Unlike laser, energy released from radio frequency devices is not ‘chromophore’ dependent. Chromophores are the molecules responsible for the colour of your skin. This means that energy in the form of heat will not be drawn to more pigmented areas, but, rather, dissipates uniformly throughout the layers of the skin, irrespective of pigmentation. Lasers in darker skin types can create problems like hyperpigmentation (darker skin patches) or hypopigmentation (white skin patches). Your treated skin will appear slightly red and minimally swollen (if at all). This often this settles over a few hours. Occasionally your redness will last up to 24 hours, but redness is generally concealable with makeup (although it is preferable to avoid makeup after any cosmetic treatment for at least 24 hours).

Over the days that follow you may experience some light flaking. You may also notice increased skin texturing in the form of a grid like pattern from stamping. This is normal and will settle over days to weeks (typically no longer than 1-2 weeks). 7 days prior to your treatment, it is recommended that you stop all active products containing AHA/BHA and vitamin A derivatives.

Should I Modify My Medication Before The Scarlet S Treatment?

If you are particularly concerned about bruising, stop fish oil, aspirin, anti-inflammatories, and blood thinners for one week prior to your treatment (if your specialist approves). However, bruising is unlikely given that heat from radiofrequency energy tends to coagulate bleeding.

How Does The Excel V+ Laser Work At The Dr. Jodie Clinic In Melbourne?

The Excel V+ laser is a fast becoming the gold standard treatment in dermatology for vascular conditions, including broken facial capillaries, generalised redness, facial flushing, and rosacea, to name a few. It  can also be used to treat brown spots, such as age spots and sunspots, and generalised skin pigmentation, and help undo the signs of photo-ageing.

It is essentially like having multiple lasers in one, due to the option of multiple wavelengths and multiple different patented handpieces.

Wavelengths

The Excel V+ laser delivers light energy at one of two specific wavelengths, 532nm or 1064nm, to target chromophores within your skin that lie either superficially or deep. Chromophores are the molecules responsible for the colour of your skin and are often more concentrated within your skin complaints. These include melanin (responsible for skin pigmentation or brown spots), oxyhaemoglobin (responsible for red spots and facial redness), and water, among others. Light energy is absorbed as heat preferentially by your chromophores, which are consequently selectively damaged. This leads to the breakdown of pigment and the spasming down of blood vessels. Your body instinctively removes damaged tissue and regenerates new skin with more collagen and elastin but without the unwanted colour.

Handpieces

The interchangeable handpieces make the Excel V+ laser incredibly versatile.

The CoolView™ handpiece emits either long pulse 532nm or 1064nm energy via a sizeable cooling glass interface to treat both small and large target areas of red (blood vessel related) or brown (pigment related) hues on your face, neck, décolletage or body, for a variety of skin conditions, in a variety of skin types. The Coolview™ handpiece is the mainstay of treatment for the Excel V+ laser, but treatment may entail some recovery time, in the form of transient mild to moderate redness, bruising, skin darkening, or swelling, depending on the size of the treatment area and individual factors.

The Dermastat is a fine, pen-like tracing device that can be used to treat the tiniest lesions, whether red or brown in colour, with incredible precision. Consultation

Your first step is consultation with your Dr. Jodie clinician , who will carefully assess your skin to ensure you are a good candidate for treatment. She will explore your main skin concerns and help you decide if your skin goals are best met with an Excel V+ Spot Treatment or an Excel V+ Global Treatment, (or even Laser Genesis in a combination approach). Clinical photos will be taken using advanced filters that clearly define the extent of your vascular and pigmentation issues. In darker skin types, a test patch may also be undertaken in this session, as this treatment may not be suitable for darker skin types, depending on the wavelength being utilised. Most importantly, your Dr. Jodie clinician will carefully explain what is involved in your treatment, your expected recovery, the number of sessions you might require and your costs.

Treatment - Spot Treatments and Global Treatments

Your Dr. Jodie therapist will cleanse your skin thoroughly and apply a cooling gel (unless the aim is a spot pigmentation treatment, which doesn’t require gel). She will also apply goggles to protect your eyes from the laser light.

When the laser is discharged, whether it is 532nm or 1064nm long pulse energy, you will experience a rubber band like snap against your skin. It will cause a little discomfort but is extremely manageable. The cooling interface with the CoolView™ handpiece offers significant relief and no numbing is required. Regardless, numbing gels are not recommended as they may cause vasoconstriction of blood vessels and interfere with treatment results.

Your clinician will use a combination of the Coolview™ handpiece and the Dermastat handpiece to conduct a ‘spot treatment’ of red spots (such as an isolated broken facial blood vessel) or brown spots (such as an isolated age spot) or a ‘global treatment’ that involves treating a larger target area such as your face, neck, décolletage or body, focusing on larger networks broken facial capillaries, generalised redness or more diffuse skin pigmentation.

As mentioned previously, this type of treatment may involve some recovery time, as it is possible to experience some redness, swelling, bruising, darkening of pigmentation, crusting, and so on.

It is important to understand that serial treatments of the Excel V+ laser are required for your best results. How Does The Excel V+ Laser Work At The Dr. Jodie Clinic In Melbourne?

The Excel V+ laser is a fast becoming the gold standard treatment in dermatology for vascular conditions, including broken facial capillaries, generalised redness, facial flushing, and rosacea, to name a few. It  can also be used to treat brown spots, such as age spots and sunspots, and generalised skin pigmentation, and help undo the signs of photo-ageing.

It is essentially like having multiple lasers in one, due to the option of multiple wavelengths and multiple different patented handpieces.

Wavelengths

The Excel V+ laser delivers light energy at one of two specific wavelengths, 532nm or 1064nm, to target chromophores within your skin that lie either superficially or deep. Chromophores are the molecules responsible for the colour of your skin and are often more concentrated within your skin complaints. These include melanin (responsible for skin pigmentation or brown spots), oxyhaemoglobin (responsible for red spots and facial redness), and water, among others. Light energy is absorbed as heat preferentially by your chromophores, which are consequently selectively damaged. This leads to the breakdown of pigment and the spasming down of blood vessels. Your body instinctively removes damaged tissue and regenerates new skin with more collagen and elastin but without the unwanted colour.

Handpieces

The interchangeable handpieces make the Excel V+ laser incredibly versatile.

The CoolView™ handpiece emits either long pulse 532nm or 1064nm energy via a sizeable cooling glass interface to treat both small and large target areas of red (blood vessel related) or brown (pigment related) hues on your face, neck, décolletage or body, for a variety of skin conditions, in a variety of skin types. The Coolview™ handpiece is the mainstay of treatment for the Excel V+ laser, but treatment may entail some recovery time, in the form of transient mild to moderate redness, bruising, skin darkening, or swelling, depending on the size of the treatment area and individual factors.

The Dermastat is a fine, pen-like tracing device that can be used to treat the tiniest lesions, whether red or brown in colour, with incredible precision. What Is Involved In Excel V+ Laser Treatment At The Dr. Jodie Clinic In Melbourne?

Consultation

Your first step is consultation with your Dr. Jodie clinician , who will carefully assess your skin to ensure you are a good candidate for treatment. She will explore your main skin concerns and help you decide if your skin goals are best met with an Excel V+ Spot Treatment or an Excel V+ Global Treatment, (or even Laser Genesis in a combination approach). Clinical photos will be taken using advanced filters that clearly define the extent of your vascular and pigmentation issues. In darker skin types, a test patch may also be undertaken in this session, as this treatment may not be suitable for darker skin types, depending on the wavelength being utilised. Most importantly, your Dr. Jodie clinician will carefully explain what is involved in your treatment, your expected recovery, the number of sessions you might require and your costs.

Treatment

Spot Treatment and Global Treatments

Your Dr. Jodie therapist will cleanse your skin thoroughly and apply a cooling gel (unless the aim is a spot pigmentation treatment, which doesn’t require gel). She will also apply goggles to protect your eyes from the laser light.

When the laser is discharged, whether it is 532nm or 1064nm long pulse energy, you will experience a rubber band like snap against your skin. It will cause a little discomfort but is extremely manageable. The cooling interface with the CoolView™ handpiece offers significant relief and no numbing is required. Regardless, numbing gels are not recommended as they may cause vasoconstriction of blood vessels and interfere with treatment results.

Your clinician will use a combination of the Coolview™ handpiece and the Dermastat handpiece to conduct a ‘spot treatment’ of red spots (such as an isolated broken facial blood vessel) or brown spots (such as an isolated age spot) or a ‘global treatment’ that involves treating a larger target area such as your face, neck, décolletage or body, focusing on larger networks broken facial capillaries, generalised redness or more diffuse skin pigmentation.

As mentioned previously, this type of treatment may involve some recovery time, as it is possible to experience some redness, swelling, bruising, darkening of pigmentation, crusting, and so on.

It is important to understand that serial treatments of the Excel V+ laser are required for your best results. At the Dr. Jodie clinic, we recommend a minimum of 1-2 treatments initially, but sometimes as many as 4-6, spaced 1 month apart. Maintenance treatments will also be required, as those who are prone to the development of broken blood vessels or facial redness or skin pigmentation will continue to develop these issues over time. It can be advantageous to combine a couple of Excel V+ laser treatments with a series of Laser Genesis treatments to keep such issues at bay, especially when you consider the no-downtime nature of Laser Genesis (again, please refer to its dedicated section under ‘Services’).

How does the Excel V+ work?

Firstly, it is worth mentioning that the Excel V+ is essentially like having multiple lasers in one. It sends light at two specific wavelengths, 532nm or 1064nm, to target chromophores within your skin that exist either superficially or deep. Chromophores are the molecules responsible for the colour of your skin and are often more concentrated within your unwanted skin lesions or concerns. These chromophores include melanin (responsible for skin pigmentation or brown spots), oxyhaemoglobin (responsible for red spots and blood vessels), and water, among others. Light energy from laser is converted to heat energy and selectively damages these targets because they preferentially absorb the heat energy. This leads to the breakdown of pigment and triggers blood vessels to spasm and shut down. Your very clever body then replaces any damaged tissue with new healthy tissue and new collagen without unwanted colour.

What Skin Types Are Suitable For Excel V+?

Excel V+ is best suited to Fitzpatrick skin types I – III, like most laser or light therapies, when using the 532nm wavelength, which targets the skin more superficially. However, all skin types can be treated using the 1064nm wavelength, which targets the skin at a deeper level.

To properly assess the type of treatment you are best suited to using the Excel V+ laser, it is critical to have a pre-treatment consultation with your Dr. Jodie clinician, who has been trained to the highest standard to identify your treatment priorities and plan your most effective approach to better skin.

Caution must always be exercised when treating darker skin, notably Fitzpatrick skin types IV – V+, as there is a greater risk of worsening pigmentation by causing post inflammatory hyperpigmentation (PIH). This occurs when your body responds to treatment-related heat and inflammation by making more, not less, pigment – exactly the opposite of what we are wanting!

Fitzpatrick skin types are described as follows:

  • I – Always burns easily, never tans and is extremely sensitive to the sun.
  • II – Always burns easily, tans minimally, very sun-sensitive skin.
  • III – Sometimes burns, tans gradually to light brown, sun-sensitive skin.
  • IV – Burns minimally and always tans (not recommended for Excel V+ treatment).
  • V+ – Rarely burns, naturally dark skin (not recommended for Excel V+ treatment) .
What should I expect during my treatment with Excel V+?

Consultation

Your first step is always consultation. You Dr. Jodie clinician will carefully assess your skin to ensure you are a good candidate for treatment before proceeding. During this consultation, she will explore your main skin concerns and help you decide what treatment is best – with so many options, even within the one device, it can be confusing without expert advice. Part of this process will involve clinical photography with specialised filters to highlight any vascular and pigmentation issues within your skin, which is a little confronting! She can use these filters to monitor your progress over time, too, which is super helpful. A test patch is not necessary prior to treatment but may be recommended for certain skin types.

Treatment

Once your initial consultation has been conducted, you are welcome to book your treatment appointment. In most cases, this can follow your consultation appointment on the same day, unless otherwise indicated during your assessment and, of course, pending availability.

Effective treatment always starts with clean skin. Please avoid wearing makeup or tinted skin products to your treatment appointment. Prior recent sun exposure and fake tan is an absolute contraindication, as it can significantly increase your risk of a burn. Prior to treatment, your Dr. Jodie clinician will gently cleanse your skin. To protect your eyes from the laser light, she will also cover your eyes with protective googles. You will still experience some flashing, but your retinas are protected!

When undergoing Excel V+ treatment with the CoolViewTM or Dermastat handpiece, your clinician will then apply a cooling gel which helps your skin better absorb the laser light energy. When the laser is discharged, you will experience a rubber band like snap against your skin. It will cause a little discomfort but is extremely manageable. After your treatment, your therapist will again cleanse your skin, apply a recovery lotion, and ensure you are covered in 50+ SPF sunscreen. Sun avoidance is a must!

What type of appointment should I book?

Always start with a consultation appointment with the Dr. Jodie therapist, as explained above.

If you are hoping to get ahead and book treatment on the same day, then here are some guidelines to help you:

  • If you just have a handful of spots (7 or less), whether these are red spots like broken blood vessels or pigmented spots like age spots, book an ‘Excel V+ Spot Treatment’.
  • If you are wanting to treat larger areas like your whole face, or neck or decolletage, whether this involves treatment of 8 or more spots or a larger target area of generalised redness and pigmentation, then book a an ‘Excel V+ Global Treatment’.
  • If you are certain that you cannot tolerate any downtime, your best plan is to opt for ‘Laser Genesis Treatment’. In some circumstances we can do a few spot treatments following Genesis, but obviously this will increase recovery time.
Should I Choose Excel V+ Laser Or IPL To Treatment My Skin Concerns?

At the Dr. Jodie clinic, we tend to recommend IPL for treatment of pigmentation or for rejuvenation if you are more prone to pigmentation, and the Excel V+ for vascular issues or its related Genesis function for rejuvenation if you are more prone to blood vessels. But this may not always be the case.

There is obviously overlap between the Excel V+ and the IPL, with both machines able to treat blood vessels, pigment, and photo-aged skin, which means you will need to rely on our guidance for your best outcome.

How Will I Look After Treatment?

After treatment with the Excel V+ CoolViewTM or Dermastat handpiece, it is likely you will experience:

Skin redness: Skin redness is completely normal after the absorption of heat energy and will likely settle within 1-2 days. With more intense treatments, redness may take longer to resolve.

Swelling: Swelling is typically mild and localised for isolated spot treatments, and mild to moderate for larger treatment areas, like the full face. Mostly, swelling will improve over the course of 2-7 days. Very rarely, full face treatments can result in severe ‘football face’ swelling (< 1%). In the event of significant swelling, we can use LED light and medication, like steroids, to help settle your symptoms.

Bruising: Bruising is common in the treatment of unwanted blood vessels. Bruising tends to be small, but may be multiple, and can last 2-7 days, occasionally longer.

Darkening of pigment: It is normal to see darkening or greying of pigmentation immediately post or in the days following treatment. Pigmentation will then either flake off or fade out over time, usually over the course of 7-10d on the face, but a bit longer on the body.

Crusting: Crusting or scabbing can occasionally occur. It is imperative for good healing to avoid picking at any crusting prematurely, which may increase your risk of scarring.

Skin Sensitivity: Following any laser treatment, it is normal for your skin to feel a little sensitive, even ‘sunburnt’, whether it appears red or not.

Are there any RED FLAGS I should be concerned about following treatment?

  • Blistering or a burn-like injury.
  • A cold sore outbreak.
  • A painful pimple like rash suggestive of folliculitis or skin infection.
  • Worsening pigmentation after 4 weeks.
  • Any significant concerns, especially concerns regarding scarring.
  • Severe bruising.
  • Severe swelling.
What are the risks of Excel V+?

All medical treatments, including cosmetic medical treatments, carry risk.

Fortunately, significant risks are rare. These include burns, scarring, post-inflammatory hyperpigmentation (pigmentation from heat that may be worse or more diffuse than the original), hypopigmentation (white patches due to the destruction of the pigment making factory within the target cells), infection (bacterial or herpes reactivation), severe bruising and severe swelling, among other concerns. Of note, the treatment of blood vessels around the edge of your nostrils can carry a higher risk of ice-pick scarring. It is important not to ‘chase’ these too hard in one session.

Consequently, at the Dr. Jodie clinic we take care to keep your treatment conservative and not push beyond what is considered safe.

However, it is important to be aware that despite adequate treatment, as with all cosmetic laser treatments, your skin concern may not be completely resolved. Sometimes partial treatment is all that is achievable.

It is likely that the skin concerns you are experiencing will continue over time with normal ageing. Blood vessels and pigmentation, lines and wrinkles, lax skin will always be recurrent issues. This is a good thing. It means you are alive and breathing!

How should I care for my skin after treatment?

Apply a cold compress or chilled gel pack post treatment (wrapped in a clean cover, never directly).

Avoid touching or recleaning skin for the remainder of the day or at least 6 hours.

Use a clean pillowcase to minimise infection risk.

Avoid facial contact with pets and children where possible.

Apply Aspect Dr. hyaluronic acid serum or an Aspect Dr. moisturiser (resveratrol or ultra light hydration) twice daily for 1 week and use a gentle cleanser. Avoid the application of oils which may intensify with facial heat.

Apply SPF 50+, preferably La Roche Posay, multiple times daily and avoid UV exposure at all costs (especially in the first 6 weeks following treatment).

Active skin care can be restarted after 1 week for most treatments.

Do not apply makeup for 24 hours – a mineral makeup or Lycogel is preferential if absolutely necessary but be aware of increased risk of infection.

Avoid exercise for 24 hours, avoid swimming/ saunas/ spas for 1 week.

Avoid alcohol for 24 hours.

Avoid anti wrinkle injections for 1 week and dermal filler injections for 2 week.

Avoid peels or facials for 4 weeks and consult with the Dr. Jodie clinic before undertaking any more intensive treatments first.

Avoid smoking, always! It will only make your skin worse.

If you are prone to cold sores, take over-the-counter medication from the chemist (if no contra-indications), preferably before or immediately after treatment.

How many treatments will I need?

For Excel V+ spot treatments, 1-2 sessions are typically required, but occasionally more will be needed (up to 4-6). For global treatments, 4-6 sessions are recommended. All treatment sessions should be spaced a minimum of 4 weeks apart. Often these treatments will lead on to Laser Genesis for maintenance.

Again, it is important to be aware that despite adequate treatment, your skin concerns may not be completely resolved, even with serial treatments, and that, sometimes, partial resolution is all that is achievable.

Try to remember that you will continue to age over time, so it is likely that you will experience a reoccurrence of your skin concerns over time. This is normal and to be expected. Blood vessels and pigmentation will inevitably return.

Are there any contraindications to treatment with Excel V+?

Absolute contraindications include:

  • Fake tan and recent sun exposure.
  • Pregnancy or trying to conceive.
  • Current or recent (within 2 weeks) cold sore (HSV).
  • Current treatment for skin cancer or other forms of cancer.
  • Current breach of skin barrier in area to be treated, e.g., wound or scratch.
  • Current flare of skin condition, such as eczema, psoriasis, inflammatory acne in area to be treated.
  • Vitiligo – an autoimmune condition causing hypopigmentation.
  • Melasma – hormonal pigmentation that is best treated in a specialist setting.
  • Past post inflammatory hyperpigmentation (PIH) – when your body produces excess pigmentation in response to inflammation/ heat.
  • History of keloid or hypertrophic scarring.
  • Current use of light sensitive medications, including Roaccutane, tetracyclines, etc.
  • Recent flare of known autoimmune condition.
  • Uncontrolled diabetes or other poorly managed chronic illness.
  • Anticoagulants, which may increase bruising risk.
  • Gold therapy, which may cause blue-gray discolouration.
  • Metal or electronic implant within 6 inches of treatment area.

How Does IPL Treatment Address Skin Concerns In Melbourne’s Dr. Jodie Clinic?

The Medi Pulse Pro XL IPL system is a non-invasive and non-ablative device which delivers intense pulses of light energy to the skin. The light energy is absorbed by molecules known as chromophores within our target skin cells. Chromophores are responsible for the colour that you see in your skin and the skin lesions you wish to treat. They include melanin (in pigment), haemoglobin (in blood vessels), and water, amongst others. The chromophores absorb the light energy as heat and the target cells are damaged. Your body then naturally removes the damaged tissue and generates new tissue in its place, without excess skin colour and with improvements in skin texture.

The Medi Pulse Pro XL is a particularly sophisticated IPL which also doubles as a BBL-like device. BBL technology allows for energy to be delivered using a gliding motion rather than a stamping motion, which allows heat energy to build more slowly and dissipate more easily. As a result, the surrounding ‘bystander’ tissue is not heated or ‘damaged’ as much when targeting chromophores, which means the treatment is more comfortable and less likely to cause complications from thermal injury. Consultation

Your first step at the Dr. Jodie clinic is always a consultation. During your appointment, your Dr. Jodie clinician can explore your skin concerns and assess whether you are a good candidate for IPL. She will determine if your treatment should involve a skin pigmentation or vascular focus or a general rejuvenation focus. Clinical photography using various filters to highlight your skin changes will be undertaken to help identify your main skin issues. A test patch will also be performed to further assess suitability and help determine your treatment settings.

Treatment

Once your treatment plan is formulated and your test patch is completed, your first treatment appointment can be undertaken.

At your treatment appointment, your clinician will gently cleanse and prepare your skin, including the application of a cool, conductive gel. You will be required to wear protective goggles throughout your treatment, but your clinician will talk to you, so you always feel in control and informed.

Depending on the focus of your treatment, you may undergo a ‘spot treatment’ of a localised area or isolated skin lesion, a ‘global treatment’ of your entire face or neck or décolletage, or a ‘rejuvenation treatment’, which is typically a lower energy treatment with a whole face approach.

Spot Treatment

During a spot treatment, your Dr. Jodie clinician will target a handful of brown spots or occasionally red spots and you can expect to feel a rubber band like flick and mild to moderate discomfort and heat. This type of treatment will entail some recovery time, as your pigment will initially darken and then eventually lift off or fade out. Similarly, a vascular lesion may bruise up, however, most vascular lesions with be treated with the Excel V+ laser at the Dr. Jodie clinic as this is considered the gold standard laser for blood vessel disorders.

Global Treatment

During a global treatment, your clinician will treat a larger area of skin, including your whole face, neck or décolletage, for generalised pigmentation or occasionally generalised redness. Mostly this treatment will be conducted using the device’s BBL-like gliding mode and you can expect to feel a more gradual build-up of heat in the treated tissue. Sometimes, a stamping mode will be necessary, which will feel more like a spot treatment, with more intense heat and a rubber band snap. A global treatment, like a spot treatment, will entail some recovery time.

Rejuvenation Treatment

During a rejuvenation treatment, your clinician will again treat a larger target area for collagen and elastin stimulation, smoother skin texture and more uniformity of skin colour. This treatment will only ever be conducted using the device’s BBL like gliding mode, but at much lower energy than a global treatment. How Does IPL Treatment Address Skin Concerns In Melbourne’s Dr. Jodie Clinic?

At the Dr. Jodie clinic, we only use the latest, state of-the-art technology. The Medi Pulse Pro XL IPL system is a non-invasive and non-ablative device which delivers intense pulses of light energy to the skin. The light energy is absorbed by molecules known as chromophores within our target skin cells. Chromophores are responsible for the colour that you see in your skin and the skin lesions you wish to treat. What Skin Conditions Can IPL Treat At The Dr. Jodie Clinic In Melbourne?

IPL is effective at treating brown spots, such as sunspots, age spots, freckles, and other forms of skin pigmentation, such as red-brown dyschromias (patchy or diffuse red-brown hues).

IPL can also treat photo-aged skin to help minimise the appearance of fine lines and wrinkles and improve the quality of your skin texture.

It is can even treat vascular conditions, including broken facial capillaries, spider veins, and other red spots, as well as generalised facial redness and rosacea.

What Is Involved In IPL Treatment At The Dr. Jodie Clinic In Melbourne?

Consultation

Your first step at the Dr. Jodie clinic is always a consultation. During your appointment, your Dr. Jodie clinician can explore your skin concerns and assess whether you are a good candidate for IPL. She will determine if your treatment should involve a skin pigmentation or vascular focus or a general rejuvenation focus. Clinical photography using various filters to highlight your skin changes will be undertaken to help identify your main skin issues. A test patch will also be performed to further assess suitability and help determine your treatment settings.

Treatment

Once your treatment plan is formulated and your test patch is completed, your first treatment appointment can be undertaken.

At your treatment appointment, your clinician will gently cleanse and prepare your skin, including the application of a cool, conductive gel. You will be required to wear protective goggles throughout your treatment, but your clinician will talk to you, so you always feel in control and informed.

Depending on the focus of your treatment, you may undergo a ‘spot treatment’ of a localised area or isolated skin lesion, a ‘global treatment’ of your entire face or neck or décolletage, or a ‘rejuvenation treatment’, which is typically a lower energy treatment with a whole face approach.

Spot Treatment

During a spot treatment, your Dr. Jodie clinician will target a handful of brown spots or occasionally red spots and you can expect to feel a rubber band like flick and mild to moderate discomfort and heat. This type of treatment will entail some recovery time, as your pigment will initially darken and then eventually lift off or fade out. Similarly, a vascular lesion may bruise up, however, most vascular lesions with be treated with the Excel V+ laser at the Dr. Jodie clinic as this is considered the gold standard laser for blood vessel disorders.

Global Treatment

During a global treatment, your clinician will treat a larger area of skin, including your whole face, neck or décolletage, for generalised pigmentation or occasionally generalised redness. Mostly this treatment will be conducted using the device’s BBL-like gliding mode and you can expect to feel a more gradual build-up of heat in the treated tissue. Sometimes, a stamping mode will be necessary, which will feel more like a spot treatment, with more intense heat and a rubber band snap. A global treatment, like a spot treatment, will entail some recovery time.

Rejuvenation Treatment

During a rejuvenation treatment, your clinician will again treat a larger target area for collagen and elastin stimulation, smoother skin texture and more uniformity of skin colour. This treatment will only ever be conducted using the device’s BBL like gliding mode, but at much lower energy than a global treatment. Again, you can expect to feel a gentle build-up of low heat in the treated tissue. Unlike a spot treatment or global treatment, a rejuvenation treatment is a minimal or no downtime event.

Regardless of the type of treatment you undertake, following your session your clinician will outline your expected recovery and carefully explain your aftercare.

What is intense pulsed light (IPL)?

Intense pulsed light, otherwise known as IPL, is a non-invasive skin treatment that delivers high intensity pulses of light to the skin for rejuvenation and to treat a variety of skin concerns, including pigmentation (isolated brown spots or generalised pigmentation) and vascular lesions (isolated red spots or generalised redness).

How does IPL work?

IPL emits a range of light energy which is absorbed by molecules known as chromophores within our target skin cells. Chromophores are responsible for the colour that you see in your skin and the skin lesions you wish to treat. The chromophore absorbs the light energy as heat and the target cells are damaged. Your body then naturally removes the damaged tissue and generates new tissue in its place.

IPL vascular treatments target red colours, our haemoglobin. IPL pigmentation treatments target brown colours, our melanin. IPL skin rejuvenation treatments target water molecules, old collagen, and other age-related skin changes to stimulate skin turnover. Rejuvenation treatments tend to have a broader range of wavelengths to treat a little bit of everything, including some redness, some pigmentation, and some textural skin issues, with minimal downtime.

The Medi Pulse Pro XL is a particularly sophisticated IPL which also doubles as a BBL-like device. What types of skin changes can be treated?

Skin changes that may benefit from IPL treatment include:

  • Pigmentation changes – age spots or seborrheic keratoses, freckles or lentigines, and red brown dyschromias, among other pigmentary changes.
  • Rejuvenation changes – lax or loose skin, crepey skin, unevenly textured skin, large pores, fines lines and wrinkles.
  • Vascular changes – telangiectasia, spider veins, haemangiomas, Campbell De Morgan spots, rosacea, and some vascular birthmarks.

Depending on your concern, your therapist at the Dr. Jodie clinic may suggest referral to a dermatologist if we believe treatment is more appropriate at a specialist centre.

Is IPL ablative or non-ablative?

IPL is considered a non-ablative treatment, which means there is technically no disruption to the surface layer of your skin. In other words, IPL delivers light energy that is converted to heat energy to the lower layers of your skin without harming your skin surface. This allows for faster recovery times. However, it is important to understand that you will still experience some downtime with IPL treatments, especially with isolated pigmented spot treatments or global pigmentation treatments that tend to be more intense (as opposed to rejuvenation treatments).

How does IPL differ from laser?

IPL uses filters to deliver light pulses within a range of wavelengths (or colours) to target a variety of skin concerns. Laser, on the other hand, delivers light pulses in one specific wavelength to target a particular skin concern. IPL is more like a lightbulb emitting an array of light, whereas a laser is more like a directed single beam of light.

The choice between IPL and other treatments depends on individual factors like skin type, your specific skin concern, and personal preferences. In many ways, IPL is more versatile, allowing you to treat a broader range of skin concerns in one treatment session than most lasers, however it may require more sessions to achieve the desired results.

Should I choose IPL or Excel V+ laser to treatment my skin concerns?

At the Dr. Jodie clinic, we tend to recommend IPL for treatment of pigmentation or for rejuvenation if you are more prone to pigmentation, and the Excel V+ for vascular issues or its Genesis function for rejuvenation if you are more prone to blood vessels. But this may not always be the case.

There is obviously overlap between IPL & the Excel V+, with both machines able to treat blood vessels, pigment, and photo-aged skin, which means you will need to rely on our guidance for your best outcome.

Are all IPL machines the same?

No! When it comes to IPL machines, just like anti-wrinkle injections and dermal filler treatments, you get what you pay for. A high quality (often more expensive) machine is better able to be tailored to your unique skin type and skin concerns – there is greater capacity to individualise the IPL settings for your treatment. Furthermore, a clinician who is skilled at skin assessment, with a good understanding of the range of wavelengths that best target your skin concerns, is critical. In that sense, your results are very ‘user dependent’. At the Dr. Jodie clinic, you can feel assured that your clinician has undergone high level IPL training in-house and with the device company, that includes theory, practical and safety.

What skin types are suitable for IPL?

IPL is recommended for clients with Fitzpatrick skin types I-III.

  • I – Always burns easily, never tans and is extremely sensitive to the sun
  • Red-haired, freckles, Celtic, Irish-Scots
  • II – Always burns easily, tans minimally, very sun-sensitive skin
  • Fair-skinned, fair-haired, blue-eyed, Caucasian
  • III – Sometimes burns, tans gradually to light brown, sun-sensitive skin
  • Average skin
  • IV – Burns minimally and always tans (not recommended for IPL treatment)
  • V+ – Rarely burns, naturally dark skin (not recommended for IPL treatment)

As a general rule, IPL treatments are not considered advisable for patients with skin types V – V+. In those with darker skin types, their natural skin pigmentation may absorb too much energy and cause significant blistering that runs a higher risk of scarring. Skin types V – V+ are also at a higher risk of post-inflammatory pigmentation, meaning their pigmentation may get worse in response to the inflammation created by treatment, not better.

What is involved in the treatment?

Consultation, Skin Assessment & Test Patch

Your first step is a consultation appointment with your clinician at the Dr. Jodie clinic. In this session, your clinician will explore your skin concerns and assess whether you are a good candidate for IPL or whether referral to a dermatology clinic is more appropriate. Your clinician will also determine if your treatment will involve a pigmentation (or vascular) focus or a general rejuvenation focus or a combination approach. Clinical photography using various filters to highlight skin changes will be undertaken to help determine your main skin issues. It is also recommended in this session that you undertake a patch test to further assess suitability and help determine your treatment settings.

Treatment

Once your treatment plan is formulated and your test patch is completed, your first treatment appointment can be undertaken. This can be completed on the same day as your consultation, but make sure that you book in both appointments (consultation & then treatment) when making your appointments. For some clients, your clinician may ask for a 24 hour window between your test patch and treatment, but this will be the exception rather than the rule.

At your treatment appointment, your clinician will gently cleanse and prepare your skin, including the application of a cool, conductive gel. You will be required to wear protective goggles throughout your treatment, but your clinician will talk to you, so you always feel in control and informed. Depending on the focus of your treatment, your clinician may (1) undertake a series of ‘spot treatments’ targeting mainly brown (pigmented) spots (or occasionally red spots), OR (2) undertake a ‘global treatment’ of your face, neck or decolletage for generalised pigmentation (or occasionally generalised redness), OR (3) undertake a lower energy, no or minimal downtime, rejuvenation treatment for collagen stimulation and better uniformity of skin colour.

During a spot treatment, you can expect to feel a rubber band like flick and mild to moderate discomfort and heat. During a global or rejuvenation treatment, you can expect a build-up of heat in the treated tissue.

After all treatments, your skin will feel as it would after a moderate sunburn. Following treatment, your therapist will outline your expected recovery and carefully explain your aftercare. Please remember that spot and global treatments are likely to entail some recovery time, whereas rejuvenation treatments may not.

Can I just have a spot IPL treatment?

At the Dr. Jodie clinic, you can target a handful of brown (pigmented) spots or red (vascular) spots or both. The Medi-Pulse Pro XL IPL is generally favoured for pigmentation treatment, while the Excel V+ is recommended more commonly for vascular issues. Your clinician will always help guide your choice.

What should I expect after treatment?

Following treatment, it is normal to anticipate some:

  • Heat. In the first hour following treatment your skin will feel very warm and you will experience a burning sensation, at least with a global or spot treatment. This is normal and will settle.
  • Skin sensitivity. Your skin may feel more sensitive than normal. A sunburn type look and feel may be apparent, and the treated area may be hypersensitive to heat for the next few days. It is best to avoid hot baths and showers during this time. If you are feeling uncomfortable a clean, cold compress can be applied to the area.
  • Redness. You may experience mild to moderate redness immediately following your treatment. Redness usually subsides within 24 hours. In some cases, the redness may take longer to settle but should improve every day.
  • Bruising. Treatment of vascular lesions can sometimes result in small amounts of bruising that may take 5 to 7 days (or more) to settle.
  • Darkened skin. Sunspots and freckles treated with IPL will temporarily darken and crust off. This often looks like ‘coffee granules’ on your skin. These may take 7 to 10 days to settle, occasionally longer. During this phase it is important not to pick at your pigment as it is lifting as it can lead to scarring or even infection. It is likely you will experience a transient bronzing of your skin.
  • Blistering. Although not ideal, blistering can happen in some areas where heat was maximal. This will not normally result in scarring unless you pick at it or do not follow instructions for healing. Please be careful with your skin!
  • Crusting. You may experience superficial crusting, even though IPL is non-ablative. Again, be gentle with your skin and do not pick or aggravate it any further, or you may cause scarring or infection.
Are there any RED FLAGS I should be concerned about following treatment?

  • Blistering or a burn-like injury
  • A cold sore outbreak
  • A painful pimple like rash suggestive of folliculitis or skin infection
  • Worsening pigmentation after 4 weeks
  • Any significant concerns, especially concerns regarding pain or scarring.
What are the risks of IPL?

It is important to be aware that all medical treatments, even those performed for cosmetic reasons, carry risks.

Significant risks are rare and include burns, scarring, post-inflammatory hyperpigmentation (which may be worse or more diffuse than the original), hypopigmentation (white patches due to the destruction of the pigment making factory within the target cells) and infection (bacterial or herpes reactivation) among other concerns.

Consequently, at the Dr. Jodie clinic we do not ‘push’ for results at all costs.

Conversely, it is important to be aware that despite adequate treatment, as with all cosmetic IPL and laser treatments, your skin concern may not be completely resolved. Sometimes partial treatment is all that is achievable.

Furthermore, it is likely that the skin concerns you are experiencing will continue over time with normal ageing. Blood vessels and pigmentation, lines and wrinkles, lax skin will always be recurrent issues.

What are my post treatment care instructions?
  • Avoid touching or recleaning skin for the remainder of the day or at least 6 hours – it has already been sufficiently cleaned.
  • Use a clean pillowcase to minimise infection risk.
  • Avoid facial contact with pets and children where possible.
  • Apply Aspect Dr. hyaluronic acid serum or an Aspect Dr. moisturiser (resveratrol or ultra light hydration) twice daily for 1 week and use a gentle cleanser. Avoid the application of oils which may intensify with facial heat.
  • Apply SPF 50+, preferably La Roche Posay, multiple times daily and avoid UV exposure at all costs (especially in the first 6 weeks following treatment).
  • Active skin care can be restarted after 1 week.
  • Do not apply makeup for 24 hours – a mineral makeup or Lycogel is preferential if absolutely necessary but be aware of increased risk of infection. This is not encouraged by the Dr. Jodie clinic.
  • Avoid exercise for 24 hours, avoid swimming/ saunas/ spas for 1 week.
  • Avoid alcohol for 24 hours.
  • Avoid anti wrinkle injections for 1 week and dermal filler injections for 2 weeks.
  • Avoid peels or facials for 4 weeks and consult with the Dr. Jodie clinic before undertaking any more intensive treatments first.
  • Avoid smoking (always – it is bad for you and ageing!).
  • If prone to cold sores, take over-the-counter medication from the chemist called Famvir, 3 tablets as a single dose, (if no contra-indications), preferably before or immediately after treatment.
How many treatments will I need?

To maximise the benefits of IPL treatment, at the Dr. Jodie clinic we recommend a course of 4-6 treatments, conducted at 4 week intervals. Spot treatments of localised pigmented lesions often leads to resolution of your unwanted skin concerns with fewer treatments sessions, say 1-2, but this cannot be guaranteed. Global treatments or rejuvenation treatments involving larger areas such as the whole face, neck or decolletage typically require the full 4-6 sessions for best results.

Again, it is important to be aware that despite adequate treatment, as with all cosmetic IPL and laser treatments, your skin concern may not be completely resolved, even with serial treatments. Sometimes partial resolution is all that is achievable.

Furthermore, it is likely that the skin concerns you are experiencing will continue over time with normal ageing. Blood vessels and pigmentation, lines and wrinkles, lax skin will always be recurrent issues.

This is why it is really important to take care of your skin with good skin care, sun avoidance and 50+ SPF sunscreen.

Are there any contraindications?
  • Fitzpatrick skin types IV – V+ – darker skin types that burn minimally if at all and are prone to tanning. (See prior information under suitable skin types).
  • Significant sun exposure in the last 4 to 6 weeks.
  • Spray tanning products or tinted moisturisers.
  • Tattoos or permanent makeup in the areas to be treated. Please note that cosmetic facial tattoos can be rendered BLACK if they are inadvertently treated. Please disclose any tattooing to your therapist prior to treatment.
  • Active or recently active cold sores (herpes virus).
  • Active skin condition in the target treatment areas e.g., eczema, psoriasis, inflammatory acne.
  • Recent facial skin treatments including waxing, IPL and laser hair removal.
  • Recent dermal filler treatment (within 4 weeks of treatment).
  • Recent anti-wrinkle treatment (within 1 week of treatment).
  • Currently pregnant or trying to conceive.
  • Epilepsy with sensitivity to flashing lights.
  • History of keloid scarring.
  • Pacemaker or heart condition that is not well controlled.
  • Skin cancer or currently receiving treatment for cancer.
  • Roaccutane or other photosensitising drugs (antiarrhythmic drugs, antifungal, diuretic, and some antibiotics, like doxycyline).
  • Warfarin or aspirin or recent use of other blood thinners.
  • Current flare of autoimmune conditions.
  • Active skin condition in the target treatment areas e.g., eczema, psoriasis, inflammatory acne.
  • Any impairment of skin barrier, e.g., wound or scratch.
  • Hormonal conditions, such as melasma, which may be best treated in a specialist clinic.
Are there any precautions?
  • Propensity to cold sores (herpes virus). Please consider antiviral prophylaxis with Famvir 3T as a single dose prior to or immediately post treatment.
  • Diabetes or other conditions that increase your risk of infection.
  • Metal implants or piercings, within 6 inches of the device.
  • Known but stable skin disorder e.g., eczema, psoriasis, controlled acne.
Is the treatment safe in pregnancy?

IPL treatment is not recommended in pregnancy or breastfeeding. Women who are trying to conceive are not advised to have IPL treatment.
