Jowls are the bulges of fat and loose skin that hang below your jawline on either side of your mouth. They tend to become more prominent with age. As you mature, you lose volume from your outer lower face, and everything falls forward and down. Mid face sag compounds the problem of lower face sag, with a ‘double whammy’ effect, worsening the appearance of your jowls.
If we look more closely at this process, there are numerous contributing factors in the development of jowls.
Over time, we see age-related bone loss from your jawline, causing loss of structure and support in your lower face. The angle of your jaw becomes less acute, and your jawbone shrinks in width and height, which affects how your soft tissues project and drape.
Fat loss in the outer lower face also contributes to volume loss and disruption of this foundational support. Changes in fat in the inner lower face is more complex. Some deep fat shrinks, including the buccal fat pads, while other superficial fat pads expand, including the jowl fat. However, the overall result is central lower face heaviness.
Central lower face heaviness is worsened by central mid face volume loss in the form of bone and deep fat. This reduces support for the overlying mid face superficial fat pads, which then begin to droop, worsening any lower face droop.
Everything is compounded by the presence of loose skin, which allows for even greater sag. The final unfortunate result is an ‘hangdog’ look, which can add years to your face.
